Provide exceptional phone and e-mail coordination/scheduling,follow-up with caregivers, nurses and clients, provide daily review of client cases, collaborate with nursing supervisor to ensure appropriate client coverage and schedule care providers for clients, carefully input caregiver/client/schedule data into Hometrak database, promptly notify Executive Director of any caregiver/client concerns, order supplies, and more...Some on-call evening/weekend coverage required and rotated between 3 client service coordinators. Phone and laptop provided...Requirements: Experienced health care coordinators, with ADL and/or HHaeXchange helpful, strong MS Office, excellent telephone manner and keen attention to detail. Training (2 -3 weeks) will be provided. Hours: 8 - 4, 8:30 - 4:30 or 9 - 5, with 1 hr. for lunch
Salary range: $50-$65k
